I think Nicole's head almost always looks photoshopped on because of her sharp facial angles and the way she picks more extreme angles to pose.

I don't think it's photoshopped. It can be done in front of the mirror by extending your head to the maximum then turn it or move your head to the left and right like indian dancers. It'll look like your head is floating, unattached. Like this photo.

I meant her skin and features look done-over. She certainly doesn't look like this in person.
